Will loaches swim up a UV sterilizer outlet?

I know most of my other fish have the good sense not to swim up the outlet, but do I need to make some sort of grate to keep Botia Kubotai out, or are they smart enough to avoid swimming up a pipe that goes above water level?

Anywhere to explore is fair game for any of the loach species (and others). In the case of the loaches, the tighter the better (lol).

I'd definitely protect the outlet with either a sponge or some other type of barrier.

Absolutely. They swim directly upstream . They will swim to the pump easily and pack up there so close it with a item like the inlet from an old HOA filter.
