I think this fish has an internal bacterial infection. See the blood under the skin in the tail and near the tail?
The injury or other problem just behind the gill cover might be related.
I would treat this fish with antibiotics. Kanamycin or other that will be taken in through the gills is easy to dose, and enters the fish's body to treat the problem. Medicated food is another good option.

Must I quarantine my fish? antibiotics can hurt my other loach and catfish in my tank?
Can I use opposite fungus medication or sea salt in water?

I live in Iran and access to tetra and sera products.
I have six clown loach (1year), My clown loach is so shy, how can I treatment this?

Hi
While feeding your fish/ single fish antibiotics, I would suggest moving it to another tank.
The antibiotics could be harmful to the other fish in your tank!

Tetra General Tonic in my general tank of clown loach and sick loach?
