Kuhli with skin problem

I just recently got a few kuhli loaches a few days ago. They were fine and really active the other day, but this morning I noticed that one was hiding in between the leaves with a skin injury. The other kuhlis look perfectly fine. I've so far moved it to a smaller tank by itself using the original tank water mixed with new water, and only added stress coat for medication.

The tank has been set up since January, with soil ground, driftwoods, and variety of anubias, a few sword plants and javamoss.
30 gallons, filtered with a penguin biowheel for 50 gallons.
Water temperature of 78 degF. Water changes every 1-2 weeks about 40%.
Other fish include a few endlers, gold prestella, bumblebee goby, sidthimunkis and yoyo loaches.
Water parameters: Ammonia - 0, Nitrite - 0, Nitrate - 5ppm. pH around 6.6.

Keep up the water changes in the hospital tank so the water stays as good as possible. Is this tank cycled? If not, add some of the media from the cycled filter to the filter in this tank. This will move some of the beneficial bacteria to the hospital tank to deal with the ammonia waste from the fish.

Most hurt or sick fish want to hide. A small cave or arch of driftwood might make him feel more comfortable. Make sure you can watch him, though, to see how he is doing. Keep it bare bottom. Kuhlies like to burrow, and you would not be able to monitor him.
